ANNUAL PERMISSION FORM
This form is currently being updated by Girl Scout Council. Please write the grade your daughter will be in 2018 (not 2016 as typed).
MEDICATION PERMISSION FORM
Under 'directions' please write, "As suggested on bottle".
HEALTH HISTORY FORM
PERMISSION TO RELEASE GIRL FORM
For the meeting date/location please write, "Fridays at Camille Davenport's home".
